- Source of history : patient, reliable.
- Chief complaint : high blood pressure.
Present illness
- A 32-old man known case HTN since 8 years ago (1392).
- HTN has been diagnosed incidentally(150/100 – 200/120).
- No paroxysmal HTN & hypotension episodes.
- BP not good controlled with 3 antihypertensive drugs.(Valsartan initially & then Amlodipine & HCTZ added).
